Older Adult Supportive Services
The Silver Care program offers compassionate, trauma-informed support for adults aged 60 and older. Through personalized assessments, our professional team identifies each individual’s unique needs and connects them to a wide range of supportive services. These may include assistance with transportation to essential appointments and community activities, access to nutritional support, and referrals for financial counseling, benefits assistance, and budgeting resources.
We also help clients navigate medical systems by coordinating access to home healthcare, therapy, or specialists, while providing social support through connection to companionship services, volunteer opportunities, or social programs to combat isolation. In times of urgent need, we guide individuals through challenges such as housing instability or caregiving demands, ensuring they receive the help they need when they need it most. Our team is dedicated to providing seamless access to both internal services and external programs, empowering older adults in our community to live with dignity, independence, and security.
