Older Adult Supportive Services
The Silver Care program offers compassionate, trauma-informed support for adults aged 60 and older. Through personalized assessments, our professional team identifies each individual’s unique needs and connects them to a wide range of supportive services. These include information and referrals to community resources such as financial counseling, benefits assistance and budgeting tools. Additionally, we provide intensive case management, offering ongoing support to help clients navigate complex challenges and connect to essential services.
We help clients coordinate access to healthcare, home healthcare services and specialists, ensuring they receive timely and appropriate care. For those facing urgent needs, our team offers guidance on issues like housing instability or caregiving responsibilities, ensuring clients are connected to the right resources when they need them most.
Our approach is focused on empowering older adults to live with dignity, independence and security, providing seamless access to both internal services and external programs that address their unique needs.
