Initial Assessment
We offer a professional evaluation of the clients within their current environment. Our highly trained care managers specialize in assessing older adults and those with special needs. We are dedicated to finding ways to enhance the quality of life while closely working with the family. If staying in their own homes is the desired plan, our care managers are experts at making sure the most critical areas of assessment are addressed:

Plan Development
A complete assessment allows a personalized plan of intervention to be developed utilizing all the resources available to ensure a better quality of life. Involving the older adult and their loved ones in this process guarantees a successful plan of action.
Monitoring
A personal JFS Care Manager monitors each of the clients to ensure that their needs are being met. The care manager is available seven days a week to offer support, encouragement and problem solving and to arrange needed services. Home visits can be done on a regular or as-needed basis.

- Our knowledge of the community and its resources allows us to offer guidance and direction in selecting appropriate living environments such as independent, assisted, or skilled nursing facilities.
- Our clients can take advantage of a wide range of JFS programs including individual and family counseling, bereavement groups, rabbinic services, and much more.
- We connect to government funded long term care programs including Veterans Assistance and Medicaid.
- We advocate for our client and families during hospitalization and doctor visits.
