 Family Link Care Management 

A program for older adults and the disabled

Memory loss and limitations imposed by disease, accident or bereavement can create anxiety, loneliness and despair in our elderly loved ones. These issues can lead to reduced attention to personal safety. Everyday tasks such as taking medicine, grocery shopping, cleaning, cooking and even bathing can be overwhelming.

Family Link provides unwavering support to relieve the hardships of aging, illness, or emotional turmoil. We focus on the client’s needs to create an invaluable link between your family and our community, improving the quality of life for all involved.

Initial Assessment

We offer a professional evaluation of the clients within their current environment. Our highly trained care managers specialize in assessing older adults and those with special needs. We are dedicated to finding ways to enhance the quality of life while closely working with the family. If staying in their own homes is the desired plan, our care managers are experts at making sure the most critical areas of assessment are addressed:

Safety needs ♦ Nutritional needs ♦ Activities of daily living ♦ Emotional/behavioral functioning  ♦  Cognitive functioning ♦ Social needs ♦ Quality of life

Plan Development

A complete assessment allows a personalized plan of intervention to be developed utilizing all the resources available to ensure a better quality of life. Involving the older adult and their loved ones in this process guarantees a successful plan of action.

Monitoring

A personal JFS Care Manager monitors each of the clients to ensure that their needs are being met. The care manager is available seven days a week to offer support, encouragement and problem solving and to arrange needed services. Home visits can be done on a regular or as-needed basis.

Link To Our Community

  • Our knowledge of the community and its resources allows us to offer guidance and direction in selecting appropriate living environments such as independent, assisted, or skilled nursing facilities.
  • Our clients can take advantage of a wide range of JFS programs including individual and family counseling, bereavement groups, rabbinic services, and much more. 
  • We connect to government funded long term care programs including Veterans Assistance and Medicaid.
  • We advocate for our client and families during hospitalization and doctor visits.

If arranging care has become too difficult to manage alone and you need someone to help coordinate a loved one’s medical (or other) appointments, you may benefit from the help of a professional Care Manager. Our knowledgeable, experienced staff can offer as much – or as little – assistance as desired.
